Older Adult Services & In-Home Support (OASIS)
The OASIS Program provides community-based, in-home services for individuals age 55+ who are unable to access traditional office-based care. The program emphasizes integrated care, crisis stabilization, medication safety, family advocacy, and connection to community resources.
THRIVE
THRIVE delivers intensive, flexible, and predominantly field‑based outpatient services for adults (18+) with serious mental illness or high behavioral health acuity. Services are provided in homes and community settings, at the office, and via telehealth when appropriate. The model emphasizes rapid response, crisis planning, coordinated transitions after ED/inpatient discharges, skills building, and integrated care with primary care and specialty providers.
